Skilled Nursing Unit (SNU)

Our hospital-based Skilled Nursing Unit (SNU) is a 33 bed Medicare unit. We offer "quality of life" care through interdisciplinary commitment. Our team consists of our resident and their family, nursing, Medical Director, pharmacist , psychologist, case manager, social worker, dietician, physical, occupational, speech, recreational and respiratory therapists.

At weekly team conferences, we discuss discharge plans, goals, progress and any changes in the Plan of Care and offer the resident and family time for questions, comments or concerns. We have a "generous" visiting policy and encourage family participation with an "at home" atmosphere. We include family in our educational system to enhance a safer home discharge. Most of our residents do return home. We, as a team, are focused on the totality of Gerontologic care and are concerned with all aspects.
